This is a sushi pen [shrimp].
When you knock the clip of the ballpoint pen, the small sushi on the plate above the pen rotates. The appearance is just like a "Kaiten" sushi. After the sushi disappears, soy sauce and shrimp tails remain on the plate. The sushi rice is also finely represented down to the grain of the rice.
The ability to incorporate a gimmick that operates without electricity into such a small mechanism as a ballpoint pen is a result of the high level of design technology unique to Japan.
